The default arrangement for a fetus is female. A fetus only begins to develop male sexual organs with the Y chromosome releasing testosterone. Hence individuals who are genetically XY but born with female sexual organs or incomplete male organs, and then later at puberty may begin to develop male secondary sex characteristics when the Y chromosome begins releasing testosterone. It's shockingly common. The "standard" medical procedure is sex reassignment surgery on the infant to create whole female external organs... which only causes pain and confusion at puberty when their so-called female body begins to develop as male, especially if the individual was never informed of the situation by their parents.
