So. A workaround that we could use is adding [NO_SLEEP] to the dwarf in its creature file. This would make it so that they wouldn't ever sleep, and thus no death from it. However, this would also mean we wouldn't have much of a need for beds, which would make the game easier.
My vote goes to just adding the [NO_SLEEP] token, unless you would like to just wait for Toady to fix the never ending sleep and the cave entrance problems.

I haven't played a Civilization game before, so I'm afraid I can't compare the two for you. =(
But please note, the game will remain in 2D with z-levels when you change character sets and graphics sets. It doesn't suddenly jump to awesome current-gen graphics. However, there is a utility called 3Dwarf that can take your maps and make them 3D, but you can't play them like that.
