We've all had moments where we wrote something and we accidentally jumble our words, getting something funny, I think. I've had characters hear smells, but never before have I had a character's "eyes solemnly turn a page."

Having characters take permanent status effects from scars, lost limbs, blindness, things like that, can often be a good alternative. Its enough of a punishment to make the players feel like the danger is real, without killing the character and forcing them to create a new one. Maybe instead of dyi...
