Generally people with strong gender identity disorder starting as early as their toddler years will turn out to be either homosexual or transsexuals. It is biological in function. Sometimes it doesn't show as much. This is only one example.
Be careful, though. It's believed that 15% of all men and 10% of all women have a brain of the other gender. This does not mean they are unhappy or want to cross the line. Transsexuals are generally miserable with what they are forced to be. Everything feels mentally and often physically out of place. It is no surprise that so many commit suicide.

And then there are some who don't figure it out until they're older. Pity, because, like Noodles said, it's best to start treatment early (i.e. before puberty). As for being sure, the problem isn't so much being sure, but convincing those who can provide treatment.

I'm a very strong believer in the ability for one to chose what gender they self-identify ... I find it said that Western culture is such a BLACK/WHITE MAN/WOMAN sort of society, because you have places like Thailand and India where a "Third Sex" sort of mentality is culturally very developed and strong. I believe there is even a democratic representative in India who is of the third-gender.

Transsexual.org? *clicks* Oh, that site. Hmm, it's okay. Not super on some details, but gives a nice overview. I don't think it's been updated in ages though.
There isn't really one site that has everything regarding TS. For a somewhat detailed insight in the whole topic you need to read many sites. And even then individual experiences vary, so you have to figure stuff out yourself while you progress.
